Tax file number form

We are authorised to collect your tax file number (TFN) under the Superannuation Industry (Supervision) Act 1993(SIS). Supplying your TFN is voluntary, and it is not an offence if you choose not to provide it.

Many super fund members who haven't supplied their Tax File Number (TFN) will be hit with an additional 31.5% tax on their employer contributions (including salary sacrifice).

Transfer of UK pension scheme funds application form

This form is for members who:

• wish to transfer their overseas pension into HESTA
• are required to pay tax on their assessable amount, and
• wish to nominate HESTA to pay the tax on their behalf at the Fund's concessional tax rate of 15%.
