Insurance transfer form

This form is for members of HESTA who wish to apply to transfer their existing Death and/or Lump-Sum Total and Permanent Disability (TPD) or Income Protection (some insurers may refer to Income Protection as Group Salary Continuance - GSC) insurance cover to HESTA.

Change of member details form

Tax file number form

We are authorised to collect your tax file number (TFN) under the Superannuation Industry (Supervision) Act 1993(SIS). Supplying your TFN is voluntary, and it is not an offence if you choose not to provide it.

Super fund members who haven't supplied their Tax File Number (TFN) may be hit with additional taxes on their employer contributions (including salary sacrifice).

Choice to have your Australian fund pay tax on a foreign super transfer

This form is for members who:

• are in the process of transferring their overseas pension into HESTA
• are required to pay tax on their assessable amount, and
• wish to nominate HESTA to pay the tax on their behalf at the Fund’s concessional tax rate of 15%.
