Team Excellence

This award acknowledges the achievements of primary health care teams that have demonstrated innovation by way of a collaborative and inclusive approach to their work. A team that shows a willingness to share their knowledge – contributing to a healthier future for all Australians.

Who is eligible?

Nominees must be a team of two or more people currently working in primary health care in Australia. Nominations will be judged on the team’s achievements in three key areas.

The nomination must:

  • explain the team’s innovative approach to program delivery or their initiation of a significant improvement to an existing program that has led to demonstrably better health care outcomes for patients or the community
  • provide evidence of a sharing and inclusive approach to the team’s work
  • outline adaptable and replicable outcomes that can be implemented by other service providers, contributing to a healthier future for all Australians.

Nominators must ensure that the act/achievements described in the nomination has taken place between 1 June 2015 and 2 June 2017 to be eligible.
