Terms and conditions for nominating someone for the 2017 HESTA Nursing & Midwifery Awards.

Outstanding Graduate

  1. Finalists in the Outstanding Graduate category will be invited to attend the HESTA Australian Nursing & Midwifery Awards dinner on Wednesday, 10 May 2017, in Brisbane, where the Outstanding Graduate winner will receive $10,000 in a ME EveryDay Transaction account courtesy of ME- the bank for you.
  2. All prizes will be awarded on the condition that the finalists and winner agree to accept their prize in person by attending the HESTA Australian Nursing & Midwifery Awards dinner to be held on the date listed in point 1 above. Finalist attendance at the awards dinner is not transferable.
  3. The prize for finalists includes:  2 x tickets to attend the HESTA Australian Nursing & Midwifery Awards dinner to be held on the date listed in point 1 above, 1 x economy class airfare to the capital city where the awards dinner is held from the finalists' nearest capital city, and 1 night's accommodation in a standard twin share room at a hotel nominated by the HESTA Australian Nursing & Midwifery Awards organising committee, close to the dinner venue. All transfers to and from airports will be at the finalists' expense unless otherwise stated in correspondence. A credit card will be required to cover any incidental expenses in the hotel.
  4. Each finalist in the Outstanding Graduate category will be presented with an award in recognition of their achievement. The winner will be chosen from the finalists and announced at the HESTA Australian Nursing & Midwifery Awards dinner, to be held on the date listed in point 1 above. The overall winner will be presented with an award, in recognition of their achievement, together with $10,000 in a ME EveryDay Transaction Account, courtesy of proud Awards sponsor ME- the bank for you.
  5. The prizes are not transferable or exchangeable and cannot be taken as cash.
  6. Only those nominees that make it through to finalist status will be notified by phone and email after the nomination close-off date by a representative of the HESTA Australian Nursing & Midwifery Awards. Finalist’s details such as name, employer/workplace, and other details in relation to the nomination will be released to the media prior to the awards dinner.
  7. HESTA, the HESTA Australian Nursing & Midwifery Awards and ME, reserve the right to use any entries, photographs, video, media and/or articles pertaining to the finalists and winners of this award, in relation to the awards, for future promotional purposes without any further reference, payment or other compensation to the finalists/winners. 
  8. Finalists in the Outstanding Graduate category must be a Graduate Nurse or Midwife holding a current practising certificate with the Nursing Board/Council in their State or Territory.
  9. Finalists must hold an undergraduate midwifery or nursing degree or diploma from a registered institution and must have been undertaking or have completed their graduate program (either full or part time) between the dates of 1 January 2016 to 24 February 2017 in order to be eligible and may be asked by a representative of the HESTA Australian Nursing & Midwifery Awards to verify qualifications.
  10. Finalists, and their nominators and seconders, and other referees including their employer, agree to be contacted by a representative of the HESTA Australian Nursing & Midwifery Awards to verify any information submitted and the eligibility of the nominee.
  11. Nominators must gain the consent of the nominee's employer before making a nomination and confirm this on the nomination form. Any nomination form that does not indicate consent will be disregarded. 
  12. The Judging Panel’s decision is final and made at its sole and complete discretion. No correspondence will be entered into. The independent industry-specific Judging Panel and organisers of the HESTA Australian Nursing & Midwifery Awards retains full discretion in the awarding of prizes.
  13. For every submitted nomination the nominee will receive one certificate in recognition of their achievement and outstanding contribution to the nursing and midwifery sector. Certificates will be provided to nominators for presentation to their nominee after the awards take place on Wednesday, 10 May 2017.

 

Team Excellence

  1. Finalists in the Team Excellence category will be invited to attend the HESTA Australian Nursing & Midwifery Awards dinner on Wednesday, 10 May 2017, in Brisbane, where the overall winner will receive $10,000 in a ME EveryDay Transaction Account, to go towards further developing their product, service, process or system, courtesy of proud Awards sponsor ME- the bank for you. The identified team leader will accept the prize on behalf of the team (team representative).
  2. All prizes will be awarded on the condition the finalists and winner agree to accept their prize in person by attending the HESTA Australian Nursing & Midwifery Awards dinner to be held on the date listed in point 1 above. Finalist attendance at the awards dinner is not transferable.
  3. Finalists in the Team Excellence category must be an individual nurse (either registered or enrolled) or midwife, holding a current practicing certificate with the Nursing Board/Council in their State or Territory, or a Personal Care Attendant, who is the initiator of the team's achievement or a team of individuals whose team leader is such a person.
  9. The teams' approach to the improvement of the product, service, process or system detailed in the submission must have been initiated or implemented between 1 January 2016 and 24 February 2017 inclusive to be eligible.
  10. Finalists, and their nominators and seconders, and other referees including their employer, agree to be contacted by a representative of the HESTA Australian Nursing & Midwifery Awards to verify any information submitted and the eligibility of the nominee.
  11. Nominators must gain the consent of the nominee's employer before making a nomination. Contact details for the person representing the consenting employer must be listed on the nomination form. Any nomination form that does not indicate consent will be disregarded.
  12. The Judging Panel’s decision is final and made at its sole and complete discretion. No correspondence will be entered into. The independent industry-specific Judging Panel and organisers of the HESTA Australian Nursing & Midwifery Awards retain full discretion in the awarding of prizes. 
  13. For every submitted nomination the nominated team will receive one certificate in recognition of their achievement and outstanding contribution to the nursing and midwifery sector. The certificate will recognise the team as recorded in the “Entry Name” field on the nomination. Certificates will be provided to nominators for presentation to their nominee after the awards take place on Wednesday, 10 May 2017.

 

Nurse or Midwife of the Year

  1. Finalists in the Nurse or Midwife of the Year category will be invited to attend the HESTA Australian Nursing & Midwifery Awards dinner on Wednesday, 10 May 2017, in Brisbane, where the Nurse or Midwife of the Year will receive $10,000 in a ME EveryDay Transaction Account, courtesy of proud Awards sponsor ME- the bank for you.

  10. Nominators must gain the consent of the nominee's employer before making a nomination and confirm this on the nomination form. Any nomination form that does not indicate consent will be disregarded.
