diabetes care for older Australians

work

Tracy is a finalist in the 2019 HESTA Australian Nursing & Midwifery Awards. She works at Bolton Clarke in Forest Hill, VIC

Tracy is recognised for her work in improving diabetes education and care for older Australians and for her leadership in the development of national diabetes policy.

As a Senior Clinical Nurse Advisor at Bolton Clarke, Tracy helps develop residential and home-based care services policies and procedures, including specialist diabetes services and education for Bolton Clarke, one of Australia’s largest providers of independent living services.

Her work includes mentoring diabetes educators and promoting access to health information by establishing translated materials across the organisation. This also includes developing audio or ‘talking books’ to make diabetes information more available to community groups.

Tracy has also demonstrated leadership in advancing diabetes care beyond her current role at Bolton Clarke, having previously held volunteer positions at the Australian Diabetes Educators Association, where she was a two-term president, Board Chair, and branch representative. In these roles, she made a considerable contribution to the development of national diabetes policies.

Tracy also has a private practice role in diabetes education and support within a Melbourne-based general practice. She has made a broader contribution to the nursing profession as a job representative and Branch Council member at the Australian Nursing Federation’s Victorian Branch.
